Dear Parent/Guardian,
The community and school age immunisation service (CSAIS) will be attending school on Thursday 25th April 2024 to offer year 8 pupils their HPV Vaccination.
Please find below a secure link to complete the consent form and the attached letter with vaccine information.
Forms must be completed and submitted by Tuesday 23rd April 2024. We cannot guarantee any forms submitted after this date will be accepted for the school vaccination session.
If we do not receive a completed consent form a nurse will contact you to discuss the vaccine, either before or during the school session.
