This letter contains a link to the consent form which must be completed, if you wish your child to be vaccinated, by 12pm Friday 1st July 2022. Our school URN is 145061.
If you are a parent of a Year 9 student who missed their HPV vaccination earlier in the year for any reason, you can also complete this consent and your child will receive their missed dose on the above dates.
